VCH is a medical home for our primary care patients 18 years and older. We diagnose, treat and manage care to improve our patients’ general health and well-being.

Our clinical teams work together to manage serious illnesses, infectious diseases, and chronic conditions such as asthma, hypertension, and diabetes, refer patients out for specialized treatment, follow-up on laboratory services, and track medical testing outcomes. Patients receive dental and optometry services at the same center, and can fill medications at an in-house pharmacy/dispensary.
