Becoming a Teenager/Young Adult 12-21 Years
Our pediatric and adolescent Care Teams work together with parents and guardians to monitor the child’s health care, track their growth and emotional progress, and build their knowledge and understanding of themselves and their bodies. Young patients have a safe, comfortable medical home as they grow, develop and mature through childhood into adolescence. Under the guidance of specialists, youths may transition from adolescent to adult primary care, and carry on receiving their health care at VCH.
