Dear VCH Patient at LAVC:
As of June 1, 2023, Valley Community Healthcare (VCH) will no longer operate the Student Health Center at LA Valley College, and offer students onsite access to medical care and behavioral health. However, we wish to make sure you continue to get the treatments and primary care services you need to manage any medical conditions, and invite you to make VCH’s North Hollywood and North Hills Centers your medical home. As a current patient, we can help you transition your care to one of our facilities, and help you be screened and enrolled in Medi-Cal coverage, if eligible, or reviewed for other programs such as our sliding fee scale or Family Pact for reproductive care.
If you prefer to select a physician from another clinic, please do so before June 1, 2023 to continue your current care. The risks of not continuing your medical care include, but are not limited to worsening of your health status, hospitalization, and loss of limb or life. We suggest you contact your current health plan for recommendations of physicians in their network, or the primary care providers or counseling services listed in this document. If you do not have health coverage, our Member Services staff will assist you in screening for insurance eligibility.
If you would like a copy of your medical records, please download the Medical Record Request Form (https://valleycommunityhealthcare.org/patients/patient-forms/) from our website, insert your information, sign the form, and return it to the VCH Medical Records Department in person, or via email to [email protected] Once we receive the completed form, we will send you a copy of your records within 15 days, at no charge. In the event that you are unable to obtain medical treatment, VCH will continue to provide necessary medical attention for 30 days after the June1, 2023 expiration of our operating contract.
Please come by the Student Health Center before June 1 to ask about transferring your care, how to obtain your medical records, and if you have questions about a prescription.
