Changes to Medi-Cal Delayed Until 2027
If you or someone in your family receives health coverage through Medi-Cal, you may have heard about changes coming next year. While the headlines can be confusing, understanding what is changing—and what is not—can help you prepare.
Medi-Cal provides health coverage to millions of Los Angelenos, including children, families, seniors, and people with disabilities. Unless otherwise changed by the legislature, some important changes are scheduled to take effect in 2027 that could affect how some members keep their coverage.
Eligibility Review & Renewals
The biggest change is that many adults enrolled through the Medicaid expansion will have their eligibility reviewed more often. Instead of renewing coverage once a year, many members will need to complete a renewal every six months.
This means it will be more important than ever to make sure L.A. County has your current mailing address, phone number, and email address. It is also important to open and respond to any letters requesting information. Missing a renewal deadline or failing to submit required paperwork could result in losing Medi-Cal coverage, even if you still qualify.
Work or Community Engagement Requirements
Another change required under federal law is that some adults ages 19 to 64 will be required to meet work or community engagement requirements to maintain Medi-Cal coverage. People may be able to meet these requirements through employment, education, job training, or volunteer service. Federal law also provides exemptions for certain individuals, including people who are pregnant or have qualifying disabilities.
Key Medi-Cal Changes in California’s 2026–27 State Budget
California’s 2026–27 state budget also includes changes that affect Medi-Cal. Beginning July 1, 2027, California will end full-scope Medi-Cal coverage for certain adults whose eligibility was based solely on immigration status. Children, pregnant individuals, and several other groups will continue to receive coverage under current state law. The state also delayed previously proposed changes to Medi-Cal asset limits until July 2027.

Keep your contact information up to date, read every notice you receive, and respond as quickly as possible if additional information is requested. If you are unsure about a letter or renewal notice, ask for help before the deadline passes.
